How to Order your LifeGem®

If you have just lost a loved one, please contact us and we will direct you to your nearest LifeGem certified funeral home. Our partners are trained to make all of the necessary LifeGem arrangements for you.

We also understand that your LifeGem(s) may be just one part of your overall memorial service.  This is why we encourage you to discuss your decision of creating a LifeGem with your chosen funeral service provider.  Our business partners can answer all of your questions.  If you have not yet chosen a funeral home, please click here to find one familiar with the LifeGem in your area.

How to Order your LifeGem® From Previously Cremated Remains

#1. If your loved one has previously been cremated, fill out this order form online with your complete contact information, including the name of your deceased loved one. You will still need to print it out and send it in with your loved one's remains.  (To just print out a form and fill in manually, click here.  Both forms require Adobe Acrobat.  You can download it free here.)

#2. On the same form, select your unique LifeGem specifics, including quantity, size, cut, etc...  Then, complete the payment information section and any special delivery instructions or notes.

#3. On page two, review the LifeGem order terms and conditions. Then, sign the authorization line and date.

#4. Separate out approximately 8 ounces (~200 grams) of the cremated remains and tightly secure in a plastic bag or other plastic container. If you do not feel comfortable separating the remains, please contact one of our certified partners for assistance.

#5. In a small shipping box, place the following three items:
a.) the sealed, separated remains, b.) the completed and signed order form, and c.) the 50% deposit if paying by check. Package as you would any other valuable item.

DO NOT send all of the remains. Due to purification, what we receive will not be returned.
(OPTIONAL - Insure your package for the amount you feel necessary.)


#6. MAIL via registered US MAIL, return receipt requested to:
LifeGem (Attn: Orders), 836 Arlington Heights Rd. #311, Elk Grove Village, IL, 60007
(Legally, ONLY US Mail accepts remains, please do not send via FedEx or UPS.)

What Happens Next?

Once we have received your order as specified, we immediately assign your unique 16 digit tracking number to ensure traceability through every step of our process. You will receive a confirmation letter that includes your tracking number for easy reference should you need to contact us.

As your loved one’s carbon proceeds through our process and nears completion, we will notify you of the progress and set up your delivery date.
